General Information

Registration for January and February 2010

1. Register online at www.vistaclinic.co.za .
2. Request that your bank faxes proof of payment to (086) 651-3172 or e-mail booking@vistacpd.co.za by the closing date the Friday before each seminar.
3. Registration will only be completed once registration and payment is received before the closing date.
4. No bookings nor payments are available on the day of the seminar.
5. No telephonic registration can be accepted.

Seminar fees
CPD seminars which are 2 hours in duration [09:00 – 11:00] - R200.00 including Vat
CPD seminars which are 4 hours in duration [09:00 -13:00] - R300.00 including Vat

Please note
Registration will only be accepted on payment of the full seminar fee before the closing date.

No bookings or payments on the day of the seminar are available.
Closing date is at 16:00 the Friday before each seminar.
No telephonic registration will be accepted.

Cancellation and refunds
All cancellations are to be in writing and addressed to booking@vistacpd.co.za 2 [TWO] days prior to the CPD that was registered for.
No refunds will made in the event of cancellation after the 2 [TWO] day period, but delegate-substitutions will be permitted. The new delegate must forward a registration form and proof of booking@vistacpd.co.za
In the case of non-arrival – no refunds will be made.

Certificate of attendance
On conclusion of each seminar attendees must submit the attendance/feedback form before vacating the venue. A certificate of attendance is obtained after each seminar.
